Applicant or representative may contact the CCC to update the application information at anytime. However, every two years the applicant or representative will be notified via mail to confirm their wish for us to retain the application. They should review and update information and return the signed cover letter and accompanying application within 30 days.

You may stop by the Comprehensive Care Center, Monday through Friday, 8:00am - 4:00pm and visit with the Admissions Coordinator. She would be happy to answer any of your questions and assist you with completing an Admission Application.

Once your application is received, a confirmation of receipt will be mailed to you. All Admission Applications remain CONFIDENTIAL.
